Abstract:

Introduction: Emergencies are unforeseen and unpredictable situations. In these situations, people’s performance is affected by various factors that cause stress. People’s performance in such situations can also affect human error probability. The purpose of this study was to evaluate human error in emergency situations based on the fuzzy CREAM and Fuzzy Analytical Hierarchy Process (FAHP). Material and Methods: This descriptive-analytical study was performed in a petrochemical industry in Markazi province in 2019. The FAHP was used to prioritize emergency situations. To evaluate human error in these conditions, the weights of Common Performance Conditions (CPC) was determined using Analytical Hierarchy Process (AHP) method. Human error probability was calculated using a fuzzy CREAM method in the most important emergency situations. Results: The results of the FAHP showed that “Hydrogen leak from the cylinder joints in the olefin unit” was the most important emergency. The highest relative weight was related to crew collaboration quality (0.06) in the emergency situation. Conclusion: This method can also be used to identify the important factors in human error occurrence and high weighted CPCs and plan to control them.

Power plant project risk assessment using a fuzzy-ANP and fuzzy-TOPSIS method

Economic growth in developing countries has resulted in increasing demand for infrastructure projects like power plants. To meet these development needs, the government of Iran has engaged companies to carry out power plant projects. While many papers have been written on the subject of project risk management, little information exists on the actual use of risk management in practice. The prim...

Urban network risk assessment using Fuzzy-AHP and TOPSIS in GIS environment

Risk assessment of urban network using traffic indicators determines vulnerable links with high danger of traffic incidents. Thus Determination of an appropriate methodology remains a big challenge to achieve this objective. This paper proposed a methodology based on data fusion concept using Fuzzy-AHP and TOPSIS to achieve this aim. The proposed methodology tries to overcome two main problems,...

Error assessment in man-machine systems using the CREAM method and human-in-the-loop fault tree analysis

Background and Objectives: Despite contribution to catastrophic accidents, human errors have been generally ignored in the design of human-machine (HM) systems and the determination of the level of automation (LOA). This paper aims to develop a method to estimate the level of automation in the early stage of the design phase considering both human and machine performance. Methods: A quantita...

Construction projects selection and risk assessment by fuzzy AHP and fuzzy TOPSIS methodologies

Construction projects are initiated in dynamic environment which result in circumstances of high uncertainty and risks due to accumulation of many interrelated parameters. The purpose of this study is to use novel analytic tools to evaluate the construction projects and their overall risks under incomplete and uncertain situations. It was also aimed to place the risk in a proper category and pr...

Organizational strategy development in distribution channel management using fuzzy AHP and hierarchical fuzzy TOPSIS

Distribution channel management not only consists of choosing distribution channels. In fact, probably the most difficult phase of the distribution management starts after this choice. Determining an appropriate organization strategy for distribution channel management is like a problem of concern to marketing practitioners and academics as well in this phase. In this study, the organization st...

Choosing shaft sinking method by using the combination of Fuzzy AHP &Fuzzy TOPSIS approaches

Shaft sinking is one of the main parts in underground mine planning. Shaft has various applications in underground mines e.g. minerals exiting flow, gases entrance or exit way, stope developments for exploitation etc. Considering that this section requires an abundance of expenses, management always following the election is the way that both terms have the technical considerations, economic co...
